REGISTER?

As a general rule, if you wish to join our list you should be residing in our practice area. Our reception staff will be happy to assist you with any enquiry you may have in this regard.

There a 2 forms that you should complete prior to joining the practice. One is a "Registration Form" and the other is "Health Questionnaire".

You can get these forms from the reception or alternatively you can download them here: Download Registration Form, Download Health Questionnaire.

Please remember that the forms must be returned to the practice in person before we can accept you in to our list. You may also require providing us with further documentations such as student cards, letter of employment, and European health card in support of your application. Again our reception staff will be happy to assist you.
